Opening Wedge High Tibial Osteotomy, Microfracture, and Bone Marrow Aspirate Concentrate for Treatment of Varus Deformity and Osteoarthritis of the Knee

چکیده
Osteoarthritis in the setting of varus deformity can be difficult to treat in young and middle aged patients. Treatment options typically include arthroplasty versus joint preservation techniques. In young and middle aged patients, unicompartmental or total knee arthroplasty is often considered a less desirable treatment option due to the increased likelihood of requiring revision arthroplasty later in life. Joint preserving techniques, which combine a high tibial osteotomy to restore a neutral or even overcorrected mechanical axis with a cartilage resurfacing or transplantation procedure, are typically preferred. Recently, some researchers have advocated for biologic augmentation of cartilage resurfacing techniques using autologous stem cells. The purpose of this report is to present two patients with a varus mechanical axis deformity and extensive grade IV medial compartment osteoarthritis treated with a medial opening wedge high tibial osteotomy (HTO) and microfracture augmented with bone marrow aspirate concentrate (BMAC). Results of these two cases demonstrate restoration of a neutral mechanical axis, filling of the osteochondral defect observed on second look arthroscopy, and improvement in subjective outcome scores. While short-term results appear encouraging in this and other reports, long-term follow-up is needed to evaluate the efficacy of joint preserving techniques in patients with large osteochondral defects for delaying or preventing conversion to arthroplasty. Combination of bone marrow derived cells transplantation and high tibial osteotomy in early osteoarthritis of knee: A preliminary study
Purpose: high tibial osteotomy (HTO) is a recommended treatment for medial compartment knee osteoarthritis. Newer cartilage regenerative procedures may add benefits to the result of HTO. In this prospective study we investigate safety and also results of HTO associated with bone marrow derived cells (BMDC) transplantation in relatively young and middle aged active individuals with early osteoar...
متن کاملThe Clinical Outcome of Simultaneous Lateral Closed- Wedge Distal Femoral Osteotomy and Anterior Cruciate Ligament Reconstruction in the ACL-deficient Knees with Symptomatic Femoral Varus Deformity
Background: Nowadays combined high tibial osteotomy and ACL reconstruction is accepted as a safe and effectivesurgery for patients with symptomatic varus osteoarthritis and anterior knee instability; however, the source of varusdeformity is sometimes the femoral bone. No studies have reported concomitant ACL reconstruction and distal femoralosteotomy in ACL-deficient knees wit...

Background: High tibial open wedg osteotomy is one of the most important modality for treatment of varus deformity in order to correct deformity and improving signs and symptoms of patients with primary degenerative osteoarthritis. The aim of this study was to investigate the results of high tibial open wedge osteotomy in patients with varus deformities. Methods: This retrospective study conduct...
متن کاملHigh tibial osteotomy.
High tibial osteotomy is effective for managing a variety of knee conditions, including gonarthrosis with varus or valgus malalignment, osteochondritis dissecans, osteonecrosis, posterolateral instability, and chondral resurfacing. The fundamental goals of the procedure are to unload diseased articular surfaces and to correct angular deformity at the tibiofemoral articulation. Although the clin...
متن کاملMid-term outcome of opening-wedge high tibial osteotomy for varus arthritic knees.
Gonarthrosis in the relatively young and active population causes major daily discomfort and disability. If the arthritic process is mainly limited to the medial compartment, the axis of a varus knee can be realigned laterally with high tibial osteotomy to unload the medial compartment and allow some cartilage regeneration and pain relief. This study describes the outcomes of patients who under...
